The appendices of the Guide to Monitoring & Reporting Voluntary Medical Male Circumcision (VMMC) Indicators are part of a resource developed for partners implementing VMMC programmes supported by the US President's Emergency Fund for AIDS Relief (PEPFAR) that may also be useful to others seeking to improve the monitoring of VMMC services. The appendices include reference sheets on the PEPFAR indicators, sample data collection and consent forms, and a checklist for assessing direct US government support for service delivery.
